Sciatica Treatment in Melrose Park | Spine Chiropractic

Evidence-Based Chiropractic Care at Spine Chiropractic Melrose Park

Sciatica is one of the most common causes of pain that radiates from the lower back into the buttock and down the leg. It can range from a mild ache to severe, sharp or burning pain that makes sitting, walking or even sleeping uncomfortable.

Many people assume sciatica is a diagnosis in itself, but in reality sciatica is a symptom. It occurs when the sciatic nerve—the largest nerve in the body—becomes irritated or compressed somewhere along its course.

At Spine Chiropractic Melrose Park, Dr Nasim Vesalpour provides evidence-based assessment and personalised chiropractic care for people experiencing sciatica. Every treatment begins with a comprehensive examination to identify the likely cause of your symptoms and determine the most appropriate management plan.

Our goal is not simply to reduce pain, but to help you understand why your symptoms developed and support your long-term recovery.

What Is Sciatica?

The sciatic nerve begins in the lower spine, travels through the buttocks and continues down the back of each leg to the foot.

When this nerve becomes irritated or compressed, it may produce symptoms anywhere along its pathway.

Although many people use the term "sciatica" to describe any leg pain, true sciatica usually follows the path of the sciatic nerve and is often associated with nerve irritation originating in the lower back.

Sciatica may affect one leg or, less commonly, both legs.

Common Symptoms of Sciatica

Symptoms vary from person to person and may include:

  • Pain travelling from the lower back into the buttock

  • Pain radiating down the back or side of the leg

  • Burning pain

  • Sharp or shooting pain

  • Tingling or pins and needles

  • Numbness in the leg or foot

  • Muscle weakness

  • Pain that worsens with prolonged sitting

  • Difficulty standing from a seated position

  • Pain when coughing or sneezing in some cases

Some people experience only leg pain with very little back pain, while others notice both.

What Causes Sciatica?

Sciatica has many possible causes. A thorough assessment helps identify which structures may be contributing to your symptoms.

Common causes include:

  • Lumbar disc bulge or herniated disc

  • Degenerative changes in the spine

  • Spinal stenosis

  • Facet joint irritation

  • Piriformis syndrome

  • Muscle tightness around the pelvis

  • Pregnancy-related changes

  • Sports injuries

  • Repetitive lifting

  • Previous lower back injuries

Not every person with a disc bulge experiences sciatica, and not every case of sciatica is caused by a disc injury. This is why an individual assessment is so important.

Risk Factors

Certain factors may increase the likelihood of developing sciatica, including:

  • Prolonged sitting

  • Repetitive bending and lifting

  • Physically demanding occupations

  • Poor lifting technique

  • Reduced physical activity

  • Obesity

  • Pregnancy

  • Previous spinal injuries

  • Age-related changes to the spine

  • Smoking

How We Assess Sciatica

At Spine Chiropractic Melrose Park, every patient receives a detailed assessment before treatment begins.

Your consultation may include:

  • A discussion about your symptoms and medical history

  • Neurological examination

  • Muscle strength testing

  • Sensation testing

  • Reflex assessment

  • Orthopaedic testing

  • Functional movement assessment

  • Postural assessment

  • Review of previous MRI or X-rays where available

If your symptoms suggest that additional investigations or referral to another healthcare provider are appropriate, this will be discussed with you.

Evidence-Based Chiropractic Care for Sciatica

Treatment is always tailored to your individual presentation and diagnosis.

Depending on your assessment findings, your management plan may include:

  • Gentle chiropractic adjustments where clinically appropriate

  • Joint mobilisation

  • Soft tissue therapy

  • Exercise-based rehabilitation

  • Neural mobility exercises

  • Core strengthening

  • Postural advice

  • Ergonomic recommendations

  • Education about pain and recovery

The aim of treatment is to improve movement, reduce mechanical stress where appropriate, support normal function and help you return to work, sport and everyday activities.

Where necessary, we work collaboratively with your GP or specialist to ensure you receive the most appropriate care.

Dr Nasim's Clinical Insight

One of the most common misconceptions I hear is, "I have sciatica because my disc has slipped."

In reality, discs do not "slip," and sciatica can have several different causes. Many patients are surprised to learn that muscle tightness, spinal joints, inflammation or narrowing around the nerve can also contribute to their symptoms.

This is why I never recommend treatment based solely on an MRI or X-ray. Your scan is only one part of the picture. Understanding your symptoms, movement, strength and neurological findings is equally important in developing the right management plan.

Home Advice for Sciatica

Many people with sciatica benefit from:

  • Remaining as active as comfortably possible

  • Short walks throughout the day

  • Avoiding prolonged sitting

  • Gentle mobility exercises

  • Maintaining good posture

  • Gradually returning to normal activities

  • Following an individual rehabilitation programme

Because every case is different, home advice should be tailored to your specific condition.

When Should You Seek Urgent Medical Attention?

Although most cases of sciatica improve with appropriate management, some symptoms require immediate medical assessment.

Seek urgent medical attention if you experience:

  • Loss of bladder or bowel control

  • Numbness around the groin or saddle region

  • Progressive weakness in one or both legs

  • Severe trauma

  • Fever with severe back pain

  • Unexplained weight loss

  • Severe pain that rapidly worsens

These symptoms require prompt medical assessment and should not be managed with routine chiropractic care alone.

Myth vs Fact

❌ Myth

Sciatica always means you have a herniated disc.

✅ Fact

While a lumbar disc herniation is one possible cause, sciatica can also result from spinal stenosis, degenerative changes, joint dysfunction, muscle-related conditions such as piriformis syndrome, or other causes. A comprehensive assessment is essential to identify the most likely source of your symptoms.

Did You Know?

Many people think bed rest is the best treatment for sciatica. However, for many individuals, staying gently active and gradually returning to normal movement is recommended as part of recovery, unless otherwise advised by a healthcare professional.

Frequently Asked Questions

Can chiropractic care help sciatica?

Some people with sciatica related to musculoskeletal conditions may benefit from chiropractic care as part of an individualised management plan. A thorough assessment is important to determine whether chiropractic care is appropriate for your condition.

Do I need an MRI?

Not everyone requires an MRI. Imaging is usually recommended when there are concerning symptoms, significant neurological findings or when it is likely to change management. We'll discuss whether imaging is appropriate based on your assessment.

Is walking good for sciatica?

For many people, gentle walking can be helpful. However, the most appropriate activity depends on the underlying cause of your symptoms and your individual presentation.

Can sciatica go away on its own?

Many episodes of sciatica improve over time. The duration varies depending on the underlying cause and individual factors. Early assessment can help guide appropriate management and identify when further investigation is needed.

Can pregnancy cause sciatica?

Some women experience symptoms similar to sciatica during pregnancy due to changes in posture, pelvic mechanics and pressure around nerves. An assessment can help determine the likely cause of your symptoms and appropriate management options.
